Understanding postnatal depression

First of all, let us take a moment to clearly distinguish the baby blues from postnatal depression, with which it is sometimes confused. The baby blues affects between 50 and 75% of women in the first days after birth [2]. It manifests as heightened emotional sensitivity, tearfulness, and mild anxiety. These symptoms generally resolve within 2 to 6 days.

Postnatal depression is quite different: more intense, it persists for several weeks or months after birth. It can take the form of deep despondency, intense guilt, irritability, a loss of interest in the baby, sleep disturbances, and sometimes even recurring negative thoughts. Vitamin B9, also known as folates (in its natural form) or folic acid (in its synthetic form), is a water-soluble vitamin that your body cannot produce on its own. The scientific link between folates and postnatal depression

Several scientific studies have examined the link between folate levels and the mental health of new mothers. One of these studies assessed the effectiveness of food supplements in the prevention of postnatal depression [7]. The research shows that women with low folate levels during and after pregnancy are more likely to develop depressive symptoms.

A large study published in the European Journal of Clinical Nutrition followed 6,800 pregnant women [8]. The results are particularly striking: women who took folic acid supplementation during pregnancy showed significantly fewer depressive symptoms 8 to 21 months after giving birth. This long-term protective effect suggests that maintaining optimal folate levels could be a worthwhile preventive strategy against postnatal depression.

Support alongside conventional treatments

Although folate supplementation does not replace medical treatments for depression, it may enhance their effectiveness. Some research also shows that folate supplementation, particularly in its methylated form, can strengthen the action of antidepressants in certain patients. The effect is modest, but has been observed in the majority of studies [9].

A French study involving 131 women found no correlation between folate levels on the third day postpartum and depression, though this study dates from 1992 [10]. However, subsequent research has shown that the duration of supplementation (at least 6 months during pregnancy), the timing of assessment (a protective effect visible at 21 months postpartum rather than in the short term) and the form used (active methylfolate or standard folic acid) are determining factors in observing a protective effect.

Folic acid or methylated folates: not all supplements are equal

Folic acid, the synthetic form commonly prescribed, is not directly usable by your body. Your body must first convert it into methylfolate (5-MTHF), its active form, via an enzyme called MTHFR. This conversion takes place primarily in the liver. However, this transformation process is not equally efficient in everyone.
