A hormonal imbalance is generally the primary cause. A poor oestrogen/progesterone ratio can promote cramping. 

Inflammation, especially in the presence of an underlying hormonal imbalance, can cause pain and cramps. This inflammation can have various origins, including diet (too much fat, too much sugar, processed and refined foods, etc.). Fatigue and stress are also significant contributing factors.

This misconception persists, and for good reason: a great many women suffer from menstrual pain. It is often linked to a deep inflammatory state or an imbalance between oestrogen and progesterone. However, common does not mean normal! Periods should not be so painful that they prevent you from carrying out certain activities.
