How do you know if you are fertile?

Many women (and men!) fear being infertile, or unable to have children — at least without medical assistance.

When do we talk about sterility or infertility in a couple? What are the causes of infertility in women and men? How can infertility or sterility be diagnosed? What solutions might be available for a couple facing confirmed infertility?

What is infertility and sterility?

We speak of infertility when there is still a negative pregnancy test after more than one year of regular sexual intercourse (2 to 3 times per week) for more than 12 months.

We speak of sterility when the probability of achieving a pregnancy is brought to zero.

A fertile couple aged 25 has a 25% chance of achieving a pregnancy with each menstrual cycle. This probability then decreases with age.

Infertility affects 5% of women aged 25, rising to 50% at age 40.

Today in France, it is estimated that 20% of couples are affected by infertility issues, and this percentage has increased in recent years. Likely contributing factors include: the environment, endocrine disruptors, excess weight, and the age at which people have their first child.

The causes of infertility are considered to be linked to:

  • In 1/3 of cases, a problem with the woman
  • In 1/3 of cases, a problem with the man
  • In 1/6 of cases, a problem with both partners
  • In 1/6 of cases, it is unexplained.

What are the causes of male infertility?

Risk factors include age, smoking, sources of heat near the testicles, Y chromosome abnormalities, pituitary gland dysfunction, environmental factors (pesticides), and overly frequent sexual intercourse (which could reduce sperm quality). Having a male fertility test, or semen analysis, helps to assess the quality of the semen and sperm cells.

The causes of male infertility are not all known, but the most common ones are:

Risk factors for infertility in women include age, smoking, being overweight, being severely underweight, or excessive physical activity. 

Reduction in sperm count and motility.

Absence of sperm in the semen, due either to an absence of production in the testes (genetic issue, chemotherapy, radiotherapy, pituitary dysfunction) or to a problem with sperm evacuation (malformation, sexually transmitted infection, surgical intervention).

What are the causes of female infertility?

The risk factors are age, smoking, being overweight or significantly underweight, and excessive physical activity. 

The most common causes of female infertility are:

An ovulation disorder may be linked to early menopause, polycystic ovary syndrome or PCOS (see our full article on the subject), a hormonal imbalance, a chromosomal abnormality, or treatment with chemotherapy or radiotherapy

Blockage of the Fallopian tubes (between the ovaries and the uterus) may be linked to an STI, endometriosis lesions, or surgical scarring.

A problem with the uterus: poor quality cervical mucus, a fibroid or polyps

Endometriosis has been associated with infertility, but the mechanism by which it affects fertility is not yet fully understood.

How is infertility or sterility diagnosed?

It is recommended to consult a doctor after 12 months of trying to conceive (with regular sexual intercourse — 2 to 3 times per week), and after 6 months if the woman is over 35.

The doctor will begin by asking the couple about the regularity and frequency of sexual intercourse, and any treatments being taken by either partner.

They will then prescribe various tests to identify a potential cause.

  • A semen analysis to assess the content of the sperm
  • An ultrasound of the reproductive organs
  • A blood test to identify any potential hormonal imbalances
  • A karyotype to identify genetic abnormalities
  • A blood test to identify any potential hormonal imbalances
  • An ovarian ultrasound
  • An examination of cervical mucus

How do you know if you are fertile?

With the test results, the doctor will be able to determine whether there is indeed an identified cause for the couple's infertility.

It is worth noting that 10% of infertility cases remain unexplained. However, unexplained does not mean there is no cause — simply that medicine has not yet been able to identify it. The psychological barrier should not be overlooked either. It is not uncommon to see women become spontaneously pregnant during an ART (assisted reproductive technology) procedure or an adoption process.

What are the treatments for infertility?

Treatment will be tailored according to the results of tests, the couple's age and how long they have been trying to conceive. In some cases, surgery may also be necessary to address the fertility issue.

In men, medication or sometimes behavioural therapy can treat certain ejaculation disorders. In cases of oligospermia (insufficient sperm count in the semen), hormones or surgery may be prescribed.

In women, hormonal treatments for menstrual cycle disorders can be effective. In some cases, surgery may be necessary, particularly if the Fallopian tubes are blocked. In the case of endometriosis, treatments to stimulate ovulation or in vitro fertilisation may be required in order to conceive.

Depending on the results of the tests, the doctor may also suggest using assisted reproductive technology (ART pathway) and in particular:

Artificial insemination

In cases of ovulation disorder, or problems with or concentration of sperm.


In vitro fertilisation

In cases of ovarian reserve low, endometriosis, or PCOS. The man's sperm are placed in the presence of the woman's egg in a laboratory, and the embryo is then reimplanted into the uterus of the prospective mother.
